This generally happens when you are entering prices inclusive of tax. This feature is only available to WooCommerce product prices. Other prices such as shipping and the fees this plugin works with CANNOT be entered inclusive of tax. You will need to enter the prices exclusive of tax knowing that the tax will be calculated and added afterward.
For many users this is because shipping is disabled. In order to group optional fees with required fees, this plugin uses a hook that is located in the shipping loop. It’s simple to move it however. Locate the file ‘woocommerce/templates/cart/cart-totals.php’ and copy it to the following location: your-theme/woocommerce/cart.
(Note: Your theme may have already overwritten this template. It is best to check your theme first and if the file already exists, use that one.)

Every row in the fees table has three required fields: Title, Cost (flat price or percentage), and Location. Generally the issue is a missing or empty Location select box. Before you can proceed with creating fees, please visit the ‘Shipping Zones’ tab of your WooCommerce Settings page. Here you can create one or more zones as needed by clicking the ‘Add New’ button at the top of the page. These zones will specify where the given fee applies. Now when you return to the ‘Fees’ tab you will be able to see options listed under the ‘Location’ column. With one of these selected, your fees should now save properly.
